Sandy Alcantara Undergoes Tommy John Surgery

Marlins star Sandy Alcantara underwent Tommy John surgery, he announced on Instagram. The procedure, performed by Dr. Keith Meister, occurred this morning. He’ll miss the entire 2024 season.

“With a heavy heart, I want to let everyone know I had Tommy John surgery today and will miss next season,” the 2022 NL Cy Young winner wrote as part of a statement. “I am most saddened for the great Marlins fans, who were so supportive of me and the team this year. The drive to the playoffs was thrilling for all of us.”

While the Marlins hadn’t indicated that surgery was on the table, it’s not an especially surprising development. Alcantara’s season was cut short last month. He landed on the injured list on September 6 with what the team initially diagnosed as a flexor strain. That was later revised to a UCL sprain, indicating some measure of tearing within the elbow ligament. With the Fish making a playoff push, Alcantara tried to return at season’s end. He threw four innings in a Triple-A rehab outing but reported additional forearm discomfort coming out of that appearance. Miami shut him down at that point.

The Fish were able to get into the postseason even without Alcantara contributing in the last few weeks. After being swept in the Wild Card round by the Phillies, they’ll go into the offseason looking for ways to build off this year’s promise. That’ll be quite a bit more difficult without one of the sport’s preeminent workhorses.

No pitcher has thrown more innings than Alcantara’s 619 over the last three seasons. Only Gerrit Cole has taken a heavier workload going back to the start of 2019. Alcantara handily leads the majors in complete games over the last three years, finishing off 10 starts. No other pitcher has gone the distance more than six times.

The two-time All-Star combines that MLB-best volume with outstanding rate production. He posted a sub-4.00 ERA every year from 2019-22, highlighted by a sterling 2.28 earned runs allowed per nine last season. He was unanimously voted the Senior Circuit’s top pitcher. Alcantara wasn’t as dominant this year, though he turned in another effective season overall. In 28 starts, he worked to a 4.14 ERA behind a 52% grounder rate while averaging over six and a half innings per appearance.

At this time last year, the Marlins had an embarrassment of riches on the mound. That contributed to their decision to deal Pablo López for Luis Arraez in an effort to balance the roster. Miami has to be thrilled with what they received from Arraez in his first season in South Florida. While they still have a good rotation, its depth has taken some notable hits.

Trevor Rogers made just four starts, none after April, because of a left biceps strain and a partial tear in his right (non-throwing) lat muscle. Prospect Jake Eder was dealt midseason for third baseman Jake Burger. Hard-throwing righty Edward Cabrera walked over 15% of major league opponents and was demoted to Triple-A for a month midseason. He managed a 4.24 ERA behind a strong 27.3% strikeout percentage. Miami’s starting staff ranked 9th in the majors in ERA (4.10), an effective yet hardly dominant result.

Left-handers Jesús Luzardo and Braxton Garrett each topped 30 starts with sub-4.00 ERA showings. Top prospect Eury Pérez worked to a 3.15 ERA while striking out nearly 29% of batters faced in 19 outings as a rookie. They project as the top three in next year’s staff, likely followed by Cabrera and Rogers. Former #3 overall pick Max Meyer should be ready for Spring Training after missing all of this year working back from Tommy John surgery of his own.

There’s significant ceiling in that group, but only Luzardo and Garrett have shouldered a full big league workload within the last two seasons. The club could be cautious with innings counts for any of Pérez, Rogers or Meyer. Ryan Weathers and Bryan Hoeing currently stand as the top depth options.

It seems likely Miami will bring in a veteran arm to add some stability to the group. They hoped that last winter’s signing of Johnny Cueto to an $8.5MM free agent deal would lock in serviceable back-of-the-rotation innings. Cueto didn’t meet expectations and is headed back to the open market. GM Kim Ng and her staff could look for a similar addition this time around.

Alcantara will count against the 40-man roster throughout the offseason. Miami can place him on the 60-day injured list at the beginning of Spring Training. As part of the extension he signed in November 2021, he’ll make $9MM next season. Alcantara is under contract for $17MM annually between 2025-26. Miami has a $21MM option or a $2MM buyout for the ’27 season.

      Where are all the people using Sandy Alcantara as an example pitching them longer and forgetting about pitch counts and innings limits. While I don’t think anyone knows how to keep pitchers healthy I just love that random people on the internet think they have an answer to billon dollar question that teams can’t seem to answer.

      “2024 isn’t in their contention window”…..In my opinion, under the new, expanded playoff format, all you have to do is finish at .500 to contend for the second or third wild card spot. All this talk of contention “windows” may now be going out the window…And for people that think the NY Mets are in trouble for ’24, I think they’ll probably contend for a wild card spot as soon as next year……I think under the new format, all but the most hopeless(A’s, Royals)of teams can dream of making a wild card run as soon as next year.

      The similarity to “fake boobs” is only that it is very common. Unfortunately, it is a real problem and nobody has a clear understanding why it has become so common. Pitching mechanics play a significant role, but in my opinion, it is the amount of pitches thrown and innings pitched, starting as early as 8-9 years old forward. When I was a kid, we pitched for our school and in the summer. When the last game was played, that was it and we were on to other sports. Today, after the summer ball season is over, it’s all small break, then fall ball. Once that is done, it’s off season bullpens and practices in prep for coming high school & summer ball seasons. I speak from experience. My 19-year old college pitcher is about to go under the knife, because what I laid out above is my son’s story and the stories of many other young pitchers today.

        Year-round sports are definitely part of the issue. I would also argue that kids sports have grown much more advanced with technology. When I played we had the batting cages with the screen to simulate live BP. But, now you have kids before high school working with pitch trax. Specifically working at just snapping breaking balls with greater rotation and lighting up radar guns is terrible on young arms , but it’s what gets you noticed.

      Yeah Tommy Lasorda, who was amazing in other respects, rode his starters until they were damaged, but lately the Dodgers have kept their elite starters at 5-6 innings. They let Buehler pitch more than most and it cost them.

      I am a fan of Kim Ng and believe she is a top 15-20 GM in baseball. But this one I have to put partly on her and partly on last year’s manager. When they fell out of contention last year they should have shut Sandy down or at least massively reduced his innings. Might not have changed anything but we never know.

        You’re absolutely right it could have caused his injury. I tore my bicept in a silly way because I decided to continue working out when I was cold. Injuries happen randomly at times and it’s best to avoid them especially if your team is out of contention. Sure he won the CY young last year but he’ll be essentially useless for the next 2-3 years.

        Sandy was lights out last year and needed to keep pitching to solidify winning the Cy Young. Yes it’s not a championship but it still matters being voted the best pitcher in your league for a year. Also there was no indication of any injury or fatigue last year. No one can conclude in this case that last year had anything to do with an injury suffered this year for Sandy.

      Your comment is flawed..the Dodgers are no more immune to this than anyone. Just 2023 they have been missing bhueler, May , Gonsolin for elbow injuries to name a few..

          We have the bozo Angels who even after knowing Ohtani had an injury still let him hit, further straining his elbow. And on the other end we have the Dodgers, which seemingly do everything right limiting IP on their veteran pitchers

          I wouldn’t go that far. They are obviously trying to win and she has done a decent job in all.

          It does go w your comment about how stupid and stuck in their ways most of these execs are however. If you are a small market team, and want to compete year in and year out, you always have to unload your top talent at peak value. ALWAYS. I thought they were going to send him back to StL for J Walker.

    Well, this sucks. Was looking forward to him rebounding to lead the staff that has progressed so much with Luzardo, Garrett and Eury Perez, with others.

    This puts the Marlins in a held position. They can play about as well as they did this year with him having a bad year, but one less arm, and no innings eater.

      He’s been hurt, but other than that he was supposed to be a legitimate prospect from the Realmuto trade. I remember a lot of people, myself included, thought Miami might have gotten the better deal with that trade. Oh how wrong we were.

    Hate to say it but I saw this happening. You don’t do what the Marlins did. You shut him down immediately. Rip the ball out of his hand. Don’t send him out there with a bad UCL. The Marlins made the playoffs but even they had to know they weren’t going anywhere with Philly and Atlanta being on the playground. This is mismanagement. All those complete games and innings and they never skip any of his starts or move him back a few days all the things you do to protect your ace and highest paid player. It’s not like Miami is going to go out and sign Yamamoto or Nola. Meyer is a big question mark and I’d expect some regression on Garrett. Looks like Ng better ask Sherman if she can spend a few more bucks. Sucks that he’s hurt I enjoy watching him pitch.

Alcantara handily leads the majors in complete games over the last three years, finishing off 10 starts. No other pitcher has gone the distance more than six times.

    Cole averages 99 pitches per start to Alcantara’s 97. It would be instructive to see how many times each of them has thrown more than a certain number of pitches, say 110 or 115 or even 120.
