Gerrit Cole Meeting With Dr. Neal ElAttrache Today

March 14: Cole is meeting with Dr. ElAttrache today, tweets Bryan Hoch of MLB.com. Manager Aaron Boone expects more information on his status this evening, although that doesn’t ensure that the Yankees will publicly disclose an update tonight.

March 13: Reigning American League Cy Young Award winner Gerrit Cole will miss at least the first month or two of the season due to an elbow injury, report Jon Heyman and Mark Sanchez of the New York Post. Cole underwent an MRI earlier this week after experiencing difficulty recovering between spring starts and throwing sessions. The Yankees haven’t made a formal announcement on the injury or a timetable yet. Heyman reports that initial imaging on Cole’s right elbow has not detected a ligament tear, though there’s some inflammation and swelling still present. Cole’s MRI has been reviewed by multiple parties, including renowned surgeon Dr. Neal ElAttrache. Per the report, ElAttrache did not see a ligament tear but nonetheless suggested an in-person visit to personally examine Cole.

While it’s a sigh of relief that early imaging didn’t reveal a major tear, the fact that Cole is going for further testing remains an ominous sign. Inflammation can at times be substantial enough that it masks structural damage. Only time will tell whether that’s the case with Cole. Regardless, to call even an absence of roughly four to eight weeks for Cole a major blow for the team would only be stating the obvious. The 33-year-old righty is on the short list of baseball’s best and — until now — most durable pitchers. Furthermore, the Yankees’ rotation behind Cole is rife with question marks.

New York signed right-hander Marcus Stroman to a two-year, $37MM deal in the offseason, but Stroman’s All-Star 2023 season was cut short by second-half injuries — a hip issue and rib cartilage fractures. He was sporting an ERA in the low-2.00s for much of the season but was hit rocked in July while pitching through the hip injury and struggled down the stretch after returning from what wound up being an extended absence due to the rib troubles.

Lefties Carlos Rodon and Nestor Cortes were both limited to fewer than 65 innings in 2023, and neither pitched anywhere close to his 2021-22 form. Rodon’s season was particularly alarming, given that he’d pitched at Cy Young-caliber levels the prior two years and inked a six-year, $162MM deal with the Yankees just last offseason. A forearm strain and hamstring strain combined to limit Rodon to 64 1/3 innings — during which time he posted a 6.85 ERA with a 22.4% strikeout rate that was 11.5 percentage points shy of his ’21-’22 rate. Cortes, meanwhile, twice hit the IL with a strained rotator cuff in his left shoulder. His 4.97 ERA in 63 1/3 innings was more than double the 2.44 number he’d posted the year prior.

Right-hander Clarke Schmidt was the only other Yankees pitcher to make even 20 starts last year. He took the ball for a full slate of 32 games, pitching 159 innings with a 4.64 earned run average. Schmidt’s 21.5% strikeout rate was below the league average, but his 6.6% walk rate was strong and his 43.8% grounder rate, which clocked in a couple percentage points ahead of the average starting pitcher. Schmidt was too homer-prone (1.36 HR/9) but at least looked like he could give the Yankees around five competitive innings every fifth day, which could prove crucial with Cole shelved for a notable period and several injury question marks alongside him on the starting staff.

The Yankees traded away a significant amount of rotation depth when dealing Michael King, Jhony Brito and Randy Vasquez to the Padres as part of the Juan Soto trade. Remaining in-house options for the now-vacant rotation spot include young righties Clayton Beeter, Luis Gil and Yoendrys Gomez — all three of whom are on the 40-man roster. Veteran righty Luke Weaver inked a one-year, $2MM deal and was expected to serve as a swingman, but he has ample starting experience in the majors and pitched well in three September starts for the Yanks last year. Top prospects Chase Hampton and Will Warren were both invited to camp as non-roster players, but neither is on the 40-man roster.

Of course, the possibility remains that the Yankees will go outside the organization to bring in a more established arm. Blake Snell and Jordan Montgomery remain unsigned, but the 110% luxury tax the team would pay on top of either player’s salary could prove exorbitant. Snell, in particular, would also cost the Yankees their second-highest draft choice in 2024 and require them to forfeit $1MM in bonus space from next year’s international amateur free agency pool. Ownership’s appetite for signing either player will now be tested. If not them, Ken Rosenthal of The Athletic wrote last night that righty Michael Lorenzen “could be an option” for the Yankees, though it’s not clear if the two parties have had any serious talks recently.

The trade market offers at least one more marquee name, and that seems to be the Yankees’ current area of focus. GM Brian Cashman and his staff have reportedly already presented the White Sox with a new offer for top starter Dylan Cease, though they’re joined by both the Rangers and Padres in their late pursuit of the 2022 AL Cy Young runner-up. The ChiSox had previously insisted that the Yankees include top outfield prospect Spencer Jones in any trade for Cease. The Yankees have balked at that ask. The new proposal to the Sox reportedly does not include Jones, though with several teams in the mix for Cease and the Yankees now staring down a potential two-month absence for Cole, it’s possible they’ll eventually acquiesce and meet Chicago’s asking price.

          Absolutely Until NYY signs a FA or makes a trade the true seriousness of Coles injury won’t be made public.

          Tanaka and Seth Lugo (after too many WBC innings) were able to be effective with serious isssues but NOT exactly like Cole

          Best FA (non trade) value for the price I believe is Clevinger and Lorenzen.

          In house NYY candidates should be OK with NYY offense.

      Sounds like a partial tear and they’re getting a second or third opinion. Probably best off doing the surgery now so he’s ready mid 2025 than waiting to see if it heals (which it won’t) and doing the surgery then (which makes him miss all of 2025).

      When a pitcher who hasn’t pitched in 6 months is struggling to recover after his third outing of 3 innings or less speaks volumes! Seeking Multiple doctor visits speaks volumes! Truth be told Cole is injured and probably needs TJS surgery! Having Cole pitch over 200 innings per year the past 4 seasons certainly may have played a part in his current situation! With Rodon, Cortez and Stroman coming off injuries the Yankee rotation is highly questionable to say the least! In heinsight maybe the Yankees needed starting pitchers more than Soto!

Connect the dots, Bauer whistled blowed the situation and mlb didn’t like it. Where’s Trevor now? Where’s Cole heading to? Fans tends to forget, the scandal broke, mlb change rules and everything is forgot.

      Look at all the over priced signed free agent pitchers who are on the shelf at this point! Degrom, Verlander, Cole, Scherzer, Yamamoto etc. Why ownership continues to hand out these long term deals to pitchers 29 or older is just plain dumb and No pitchers should get contracts that go beyond their age 34 season period….

        I think that CERTAIN starting pitchers are a better bet through age 40 than almost ANY position players are

        Its the opposite my guy. You dont go for an in person visit if theres nothing more wrong than just inflammation. Remember this was initial imaging. There’s liking a minor tear ala Tanaka/Glasnow and hes going the for a consultation on whether he should get the surgery now or try to play through it.

      Cole will know his fate no later than 1st week of June or sooner. Either way he not going to be available until 2026 season unless he opts for surgery now and then be available end of July 2025 so its his call. One thing is certain, he is not exercising his opt out cause if he does he is a goner with Yankees off the hook for remaining 4/144. I could see Boras barking in his ear to still excercise his opt out cause some team will offer Cole a 6/ 225 deal broken down $10 mill for 2025, 2026-2030 $43 million per through his age 39 season with a new elbow.

    Worst case he gets the rest he needs, ramps up and if the elbow still barking then he gets the TJS and ends up not returning until 2026 fully healthy. No sense returning late 2025 when he wont be 100% anyways. Hopefully one of Gil, Beeter or Warren can fill the vacant hole as a #5 and Stroman or Rodon step up their game.

      I could see Stroman really balling out now that he will be more heavily relied upon. He’s had flashes of pitching like an ace at times, including most of last year.

      If Rodon gets back to his best, they will be fine. Big if though.

        Bauer still has not learned humility. Last weekend, he played for the Asian Breeze against Dodgers minor leaguers and near the end, Bauer was letting fans call out his next pitch and announcing the pitch type before he threw it. That type of arrogance does not play well from a guy who was accused of harrassment or assault by multiple victims. Look at the past tweets from Bauer attacking a college age girl, the guy has issues.

        Yes, we know that the one victim tried to set him up, and that Bauer was the stupid patsy who was easily suckered in – but that hardly amounts to him having been found innocent even in that case where he admitted to hitting her after she was unconscious. It only means that the D.A. concluded that further resources were not justified.

        Bauer has served his suspension, I have no problem with an owner who wants to sign Bauer, but as an owner, they should look at whether it will increase or harm team popularity and revenues.

          Here’s the thing, Hotdog. That fun came at the expense of other people. Something Trevor has a history of doing on his channel, and something Trevor got down on his hands and knees and apologised to MLB for not that long ago. I’m not sure what the Manager that looks at him with goo goo eyes is doing, but giving him sound, intellectually mature advice, doesn’t appear to be on the list.

    Yankees are good enough to compete without him, but I don’t think they’re going to win that division. Even with Cole that was a tough ask. Snell, Monty or Cease aren’t going to replace that production either, but it’d help.

      The lineup and bullpen are good. The rangers made it work last year with a rotation full of question marks. It’s a tough route to the WS but it can be done!

          The O’s have their own issues with Bradish, who was their best pitcher last year. They’re hoping Bradish doesn’t have to go under the knife and that Means comes back pretty soon. And the Rays have had major injuries in their rotation too. Anything could happen this year.

        at least on paper they improved more than a lot of people think. having verdugo instead of a bunch of sub-replacement hitters and a healthy rizzo moves the needle even before you add soto. if volpe and wells can take baby steps forward it’s a strong line-up. the season comes down to rodon/stroman/cortes

      I disagree. Snell could easily duplicate Cole’s wins albeit less innings and more walks but no way Yankees paying $65-80 million when hit with tax. Too risky and expensive.

        Anthony I think you’re right about both Snell’s value, and NY not wanting to pay him. Without a real Cole replacement they maybe get a wild card spot if a LOT goes right. Staying optimistic that this year isn’t as excruciating to watch as last year.

        In an 162 game average, Cole has average 211 IPs and Snell with 177 IPs. Snell isn’t always available. The 34 IPs difference isn’t insignificant bc a team will need to carry an extra elite reliever to close that gap. 177 IPs is most likely Snell’s ceiling now. Nobody can replace Cole.

          Obviously, there is no one to replace him. But the addition of another pitcher, and elite one or not, will certainly help. if Cashman thinks he’s going to solve this by his usual MO of dumpster diving, it will be a bigger problem.

    They shouldn’t allow this to force them into a bad decisions. If anything it may lead to him not opting out but regardless they aren’t that good so if they are gonna make a huge signing or trade it shouldn’t be because of this. I know how the NY market is demanding tho.

    The way I read it is like this.

    If I’m the Yankees front office, I don’t want other teams knowing we are in desperate need of a pitcher. It makes the asking price go up knowing you’re negotiating from a weaker standpoint.

    So, he might only have some inflammation…..or he might need TJS. But if I’m the front office, I wouldn’t announce that it’s as serious as it might be.

        Neither have worked out yet, and they well may be the best the Yankees have. That doesn’t mean the projections aren’t on the high end of possibility. Some teams best prospects end up being platoon players or average regulars. Projection is just another way to say educated guess.

    Man, too many people don’t understand these injuries. Swelling 1. can mask a deeper injury and 2. You cannot get the surgery done until that subsides. They should know more after the in person visit to ElAttrache, but eithervway there’s going to be a waiting period. Hoping for the best here!

      Well it might play well to actually follow the AL East before making your comment about 4th place as if that’s automatic…..offensively, the Yankees are as good as any team in the East period……and as far as pitching, read this morning that the Rays suffered another pitching injury to one of their top guys, wondering exactly what and how deep both the Jays and Orioles rotations are (the Orioles ace can’t pitch 5 days a week) and of course there’s Boston……you can go back to class for nap time now…..

        “Before making your comment about 4th place”…Baseball is not linear and is near impossible to predict over a large sample size. I’ve seen teams add a great player and somehow play worse and I’ve seen a team(Atlanta Braves) lose a great player in Acuna and somehow play better in winning a World Series. Knowing what you actually know is a skill and the people that predict baseball do not really understand baseball.

      For all that is riding on getting the diagnosis correct I don’t blame NYY for getting multiple 2nd or 3rd opion.

      (Big $, a possible deep run in the playoffs this year and upcoming, probability Soto resigns, ticket sales and much more

      As a secondary concern NYY must have some insurance on Coles contract

      It is a puzzle with many variables to sort out.

        Basically the opposite spectrum of the Red Sox, and the irony is they’ll still probably make the best trade and make it to the postseason because they’re the Yankees.

    Love these “Doctors” on here who know…nothing. He might get a PRP injection and some rest and be fine. Or, he might need full-blown TJS. Nobody knows. There is a LOT of miles on that tire however. That we do know. But blaming Brian Cashman for him “delaying” getting TJS is just silly. How about we all wait until the soufflé is out of the oven?

    An in person visit with the doctor is usually what happens after imaging, even if you’re not a pro athlete. Mine got results that contradicted the MRI and I didn’t need knee surgery. Pro athletes are a different animal than older field medics, so the exam is more likely to result in “when do you want your surgery to take place?” They’ll need a replacement either way, just a matter of how long, so why would any team tip their hand further by saying “our ace is out for almost 2 seasons”? Tough break for Cole and the Yankees, but his is the season that’s probably over, not the Yankees.

    I said from day 1 that the innings would catch up to Cole. Sustaining 200+ innings for multiple years and then the Yankees signing that mega contract… I’m sorry, but in this day and age, innings are going to catch up to him and Yankees should have known that before signing a mega contract for many years.     So, if the Detroit Tigers want to play a dangerous game of Talent Roulette involving perhaps an epic arm and perhaps a great future CF, dangle Tarik Skubal for Yankee prospects (start with demanding 1 and 2 and see where it goes).

    Frank Lane woulda done it.

    I’m talking Skubal. Not Jobe.

    Signs point (health permitting as always) to this being a dominant breakout year for Skubal. I don’t know if the present rules allow for a creative trade’s outcome, including PTBNL, to be conditioned on performance, but if so by all means let’s do that to ensure everyone winds up a winner.

    The Tigers are going nowhere as usual. If Scott Harris thought otherwise he wouldn’t have populated his roster with what he has recently added.

    The Yankees need an ace, a lefty in that park is a bonus, and they don’t want to pay Snell. But Soto, so they must forge ahead right now and replace an ace.

    Cease will not measure up to 2024 Skubal.

    The Tigers end up with one and maybe two stars locked in for years at a pauper’s wage.

    OK. There you have it. Fiction Life over. Back to ho-humdrum.

      Ummh no. Yankees prospects are magic beans Jack. The only way they should consider trading Skubal is for a bigger haul than the White Sox got for Cease. Even then, I think they should keep him. Trading him when the team has a chance to compete doesn’t make sense.

      I’m pretty happy with Harris and Greenberg’s offseason. Flaherty and Maeda have looked good, as well as most of the bullpen additions. Canha has been meh so far but Kelly has been a pleasant surprise. Keith/Ibanez and Urshela/McKinistry have looked pretty good too for the 2B and 3B spots and Meadows in CF. Even Kreidler has looked better. Platoon him and Leonard at SS and bench Baez!

      I’ll be surprised if they don’t challenge the Twins for first.

    Offense should weather the storm! Rodon nester stroman schmidt and weaver and then look to either Monty or lorenzen… snell is to much with attaching bonus pool and draft picks. Cease would be ok but would cost a ton of prospects why not try to trade a package of Everson Pereira with either Lombard or arias. To Miami for lozardo or aj puk type

    Happy Cole has a chance to join the rotation. The guy is a stud and it would not only suck for the Yankees but baseball as well. As a Yankee fan I hope they are willing to pivot and forget entertaining the idea of trading more prospects for Dylan Cease who I truly think is Sonny Gray 2.0! I hope they have the sack to do some hard research and consider Trevor Bauer and if not him being back Monty!

      @Astros_fan_in_Aus Cole is probably my favorite player in MLB, partly because he hasn’t missed a start since 2016, goes deep, knows how to actually pitch, can make adjustments, etc. So far, he’s a real throwback to that fabulous group of pitchers who debuted in the 60s. I LOVE watching good pitchers pitch.

      It’s not just burning a year of Soto, every one of the arms dealt in the Soto trade could have been necessary reinforcements, one of those arms even helped the Padres land Cease. If there is a team that needs Snell as of right now its the Yankees. After all, what better way to replace the 2023 AL Cy Young award winner than with the 2023 NL Cy Young award winner

    All of these pitching injuries makes me wonder if the pitch clock did more damage than we realize.

      It’s not the clock that’s the problem, its the “max effort” style of throwing that is causing this. Pitchers today do not actually know how to pitch, they just know how to throw as hard as possible until something pops. Look at all the hard throwing, high strikeout pitchers of the past and you’ll see some pretty lengthy career with no arm problems because they actually knew how to pitch

        Today’s pitchers know more about pitching than any generation of pitchers ever. Tunneling, wake, spin, biomechanics, etc

      I think anyone who dismisses the clock as a possible/probable contributing factor is being foolish.

      There’s also the removal of the ‘sticky stuff’, COVID short season, analytics’ obsession with K’s, more powerful muscles around joints and smaller parks/less foul ground.

      We’ll likely never know what factor contributed most to the increase in severe elbow injuries.

        Is it even that much of an increase or is this just recency bias. Arm injuries have been increasing long before the clock was a thing in MLB.

        It is likely a combination of things, but the pitch clock could be exacerbating all of those factors. Less rest time in between pitches can tire them out faster and it is fairly well known that players are more likely to injure themselves when they are tired.

      I would say it is probably too early to say definitely but I believe for as maybe a good 40% of pitchers (rough estimate) injuries might increase possibly skewing towards older pitchers who are not used to having a pitch clock coming up through the minors

      This just kind of a working theory which after another 4 years could be analyzed better. Reasonable people can disagree – goes without saying

      The pitching motion is un natural, injuries will always happen.
      Many pitchers are kind of creatures of habit in their preparation – Day pitching and days between. Relievers can’t always know how often.and managers try not to overuse (esp. closers) but they have pressure to win.

      I’m on torque. Numbers, as opposed to the eye, are now defining how much your arsenal moves in the air. As a consequence, pitchers are more focussed on chasing hard movement. More torque. More injuries. Humans don’t twist well.

    Receive plasma rich injection in elbow, rest for awhile, then slowly gear it back up w/fingers crossed, an avenue pursued by many and w/very low success rate usually leading to bein cut anyway … or just get the surgery now and be available sooner and get this behind you. There’s reason he is in LA today meeting w/1 of best Dr’s in world 4this procedure.

    Looks like the Yankees will be focus on retaining Sorou and going full throttle after Burnes this offseason. If the doctor says knife is needed then is ’till ’26 then the rotation could look like
    Burnes Cole Beeter-Strohman (if resigned) Rodón-Schmidt and Gil-Warren. Looks like a good Rot

    And why was Cole such a JERK to the guy asking him questions while walking along the sidewalk ? I was wishing the bwst for Cole but now I hope he DOES need TJ – just like ML ball players RUDE – fans are already upset with their GREED and high prices and how RUDE they can be etc – also these new baseball rules havent helped along with commercials in the middle of the game plus shoddy play as we see and less talent on the field

        I see ur point but he is a ML ballplayer and a star it comes with the territory – its not like he just got the news of the injury 5 minutes ago – but yes maybe I am abit too harsh on him but still stop and at least say I cant comment on anything the Yankees will do so

          To be fair this is likely the worst injury he has ever had. He is not used to being sidelined. All players know it won’t last forever, but that first big sign is not always easy to process. When something you love gets taken away from you it leaves a mark.

    MLB has fallen behind the NFL and NBA as popular sports – instead of adding teams like the commish states its time to disband some – Pirates – Marlins – Rockies – A’s – Brewers – Royals should be first one’s to go – what talent is on these teams go be then spread among the leftover teams and improve the level of play — this would help the pitching the most as there are so many players who should not be on a ML roster

    Yankees could have had Cease and dragged their feet – serves them right – Padres traded guys they didnt need where the Yanks worried about holding on to Spencer Jones who does appear to be anything special – I guess the Yanks dont want to win – now they can go spend on Snell – Cashman should be fired –
