Alek Manoah Headed For Second Opinion, Likely Facing Lengthy Absence

May 31: Manoah is headed for a second opinion on his elbow, reports TSN’s Scott Mitchell. His initial diagnosis isn’t yet known, but Mitchell adds that it’s expected Manoah will be sidelined for an “extended period of time.”

May 29: Alek Manoah left tonight’s start against the White Sox in the second inning. The team announced he experienced elbow discomfort. Manager John Schneider told the Toronto beat after the game that Manoah will get an MRI tomorrow (X link via MLB.com’s Keegan Matheson).

Manoah lost the first month of the season after battling shoulder discomfort during the spring. He was reinstated on May 5 and has taken the ball five times. The burly right-hander was out to an encouraging start to the season, working to a 3.70 ERA with 26 strikeouts and eight walks across 24 1/3 innings. Manoah wasn’t back to his Cy Young finalist form of 2022, but it was a substantial improvement over last year’s disappointing season.

There’s little to be done now beyond hoping that imaging doesn’t reveal any structural concerns. Manoah’s fastball sat in the typical 92-93 MPH range during the first inning. He didn’t top 90 MPH in the second inning until throwing Dominic Fletcher a 91.4 MPH sinker to start his at-bat. Manoah seemed to wince after releasing that pitch and departed the game following a mound visit (video provided on X by Sportsnet’s Shi Davidi). Trevor Richards came in for 3 1/3 scoreless innings of emergency relief to help Toronto to a 3-1 victory.

The Jays are working with limited rotation depth behind their front five of Kevin Gausman, Yusei Kikuchi, José Berríos, Chris Bassitt and Manoah. Offseason signee Yariel Rodríguez has been on the injured list since April 30 with back inflammation. Bowden Francis, who opened the season in the rotation when Manoah was on the shelf, has been out for the last month with forearm tendinitis.

Rodríguez and Francis are each on rehab stints with Triple-A Buffalo. While both pitchers should be back before long, they’ve each got questions about their viability as starters. Rodríguez barely pitched in 2023 as he went through the process of applying for free agency. He pitched out of the bullpen in Japan two seasons ago. Francis was rocked in his first two MLB starts this year and quickly kicked to multi-inning relief, where he had more success last season.

Paolo Espino, who has a 4.81 ERA over eight starts with Buffalo, is the only other traditional starter on the 40-man roster. Toronto brought back old friend Aaron Sanchez on a minor league deal earlier this month, but he has given up 17 runs in 10 2/3 Triple-A innings. Beau Sulser, another recent non-roster acquisition, has surrendered 12 runs in 14 1/3 Triple-A innings in a swing role. Top prospect Ricky Tiedemann is back on the minor league IL because of a nerve issue in his elbow.

If Manoah needs to go on the injured list, Espino seems the logical choice to hold the fort until Rodríguez is ready to return. Toronto’s rotation would look perilously thin in the event of any other injuries. While the Jays benefitted from remarkable rotation health last season,  they relied on Richards to kick off a few bullpen games last summer while Manoah was in the minors. Perhaps they’d look at that as another short-term solution, but starting pitching would likely be a deadline priority if the Jays are in the postseason picture in July.

    Did he try to stay in shape after getting demoted and turning into a toddler because of it? I wouldn’t be surprised if he was slacking off and this is the result of him actually trying after getting back to the big leagues.

      He came into spring training in February in fantastic shape with huge weight loss. You’re making things up that could easily be disproved by glancing a photo of the guy. He’s certainly in better physical shape than he was during his best season.

      But when you have a shoulder injury, that often leads to an elbow injury later because your body gets a little out of sync or you learn to favor something. Even though there were months between them it’s possible the first injury lead to the second indirectly.

        Smuz, as someone who has physically seen what this guy eats at the hotel’s he stays at for away games, even getting in “better physical shape than he was during his best season” isn’t really much. It’s lipstick on a pig.

        TJ probably doesn’t correlate to being overweight, no. There is however data that is starting to link conditioning and extended throwing past pitch and inning limits that correlates to TJ and Manoah has not been healthy enough due to injuries that could stem from unhealthy eating and weight as well as poor performance which also could be due to his poor conditioning, to benefit from extended throwing throughout his career.

      Slacking? No. Manoah has been very effective the last few starts. Exactly what the Jays needed.

      Manoah had two great starts and a third with questionable defense behind him causing his numbers to look worse than it should have been. It could have been Manoah turning the corner, which brought him back to being an effective major league pitcher – rather than a slacker as you stated.

      DFA Ross stripling he sucks – has a great 2022 in the rotation while rattling off a stretch of 9 starts where he was maybe the most dependable on that run THEN “gave bassit all that money ? He’s basically more expensive stripling “‘ we saw how those 2 compared in

      Yes except the Guardians don’t seem to know what to do with their talent much like Atkins and Shapiro. Difference is Cleveland seems to draft better, develop better, and play in a horrible division unlike the Jays.

    These comments are sickening. Here’s a guy who’s worked his tail off to get to and try to stay in MLB.. He’s been dealing with medical issues for a year now. All the same people who were glowing about him and what a great story he was when he was having great years are now delighting in his misfortune. I’m outta here,

      I don’t disagree, but it’s the same in real life. No one wants to see anyone get laid off, but there are some people that will break your heart, and some people that you’ll smirk over.

      Manaoh seems to have done a lot to earn the animosity.

        Yep, Manoah is no babe in the woods when it comes to controversy. If he does need TJS that would be an opportunity for him to mature, hone his approach and attitude, and come back better than ever. Only time will tell, but some players need a reset.

    If there was any question of whether the Jays should be sellers this settles it. Best thing for them to do is try to move Kikuchi while he’s healthy and pitching well. Need to wait for Vlad and Bo to get on a tear before moving them.

    If Manoah is done and the Jays are planning on competing for a playoff spot they are going to need to replace him. A piggyback of Rodriguez/Francis is a non-competitive option. The Jays also need hitting. They can satisfy one of these conditions, not both. The farm isn’t that deep. Fixing one won’t help. The only thing to do is start selling.

      Yea I’ve been on the sell train for awhile as I don’t thing classy is worth 300, Bo I just don’t see re signing, Jansen is too close to FA now and almost everything else is a FA by next off season. A rebuild is clearly coming and if you have Those pieces plus a good relief pitcher or 2 to trade you really really jump start a rebuild. Problem is with record payroll number 2 years running and 2 years of massive stadium investments Ownership wants as much attendance as possible and I don’t see they giving the green light

    I was hopeful for him, and in one game, he did pretty well through the first 4 innings before he got pulled. The next time I saw him pitch, the swagger had returned and his performance was off. It seems that his Come to Jesus moment has subsided.
