Shane McClanahan To Undergo Tommy John Surgery

By Anthony Franco | August 15, 2023 at 11:58pm CDT

Rays ace Shane McClanahan will require Tommy John surgery next week, reports Marc Topkin of the Tampa Bay Times (Twitter link). He’s expected to miss the entire 2024 campaign.

It’s a devastating if not entirely unexpected development. The two-time All-Star left his start on August 2 after experiencing forearm discomfort. Tampa Bay has been exploring their options for the past couple weeks, though the club had already confirmed he was expected to miss the remainder of 2023 and that surgery was a consideration. He’s already on the 60-day injured list.

Surprising or not, it’s hard to overstate the significance of losing a pitcher of McClanahan’s caliber. The former first-round draftee debuted with 25 starts of 3.43 ERA ball two seasons ago. By his second major league season, he’d broken through as one of the game’s best pitchers. McClanahan was a legitimate Cy Young contender into August last year before a shoulder impingement cost him a couple weeks.

The absence dropped the South Florida product to sixth in Cy Young balloting, but he still finished his sophomore campaign with a 2.54 ERA through 166 1/3 frames. While his ERA took a bit of a step back this season, a 3.29 mark over 21 starts still ranks 19th among big league starters (minimum 100 innings). He’s 23rd among that same group with a 25.8% strikeout rate. McClanahan earned his second consecutive All-Star nod last month.

Tampa Bay has dealt with a brutal stretch of pitching injuries over the past few months. Top prospect Shane Baz has missed the entire year after undergoing a Tommy John procedure last September. McClanahan becomes the third member of the Rays’ season-opening starting staff to require surgery this year. Jeffrey Springs required a Tommy John procedure in April; Drew Rasmussen underwent flexor tendon surgery last month.

Even after the absences to Baz, Springs and Rasmussen, the Rays had an effective top of the rotation. McClanahan, Tyler Glasnow and Zach Eflin made for an excellent trio. With McClanahan now out of the picture, Glasnow assumes the role of staff ace. Eflin — who has dealt with knee issues throughout his career but has a 3.67 ERA through 23 starts in his first season with Tampa Bay — is the #2 option.

The early-season injuries led the Rays to acquire Aaron Civale from the Guardians at the deadline. That acquisition takes on increased importance with McClanahan out. Still, there’s little recourse beyond internal options for Tampa Bay in patching together the final two rotation spots. Taj Bradley has been up-and-down as a rookie and is on optional assignment to Triple-A. The Rays have pushed Zack Littell into the rotation and recently activated Josh Fleming from the injured list.

Tampa Bay enters play Tuesday three games back of the Orioles in the AL East. They’re still in strong playoff position, 7.5 games clear of the Mariners (the top non-playoff team in the AL). Their chances of running down Baltimore to secure a first-round bye and/or making a deep run once they get into October are obviously dealt a hit by the loss of one of the game’s best starters.

This is the second Tommy John procedure of McClanahan’s career. He missed his freshman year at USF after undergoing surgery in 2016. The Rays figure to be especially cautious with his recovery as a result, although there’s a good chance he would have missed all of ’24 even if this were his first TJS.

McClanahan has just under three years of major league service. He’ll be eligible for arbitration as a Super Two player next offseason. The Rays will need to carry him on their 40-man roster during the offseason but will immediately place him back on the 60-day IL next spring. He’ll collect service time while rehabbing and remains on track to first reach free agency after the 2027 campaign.

Between Shane and Wander, must suck to be a Rays fan this week, yet they are playoff bound and Yankees and Red Sox likely are not.

Long way to go in the season. Better teams than this Rays team have imploded in September under far better circumstances. Wouldn’t surprise me a bit if they fell off a cliff, especially if Wander is out for an extended period of time.

Seattle is “only” 6 behind Tampa in the loss column. Boston is 7 behind. A big gap but not insurmountable.

The Red Sox gauntlet schedule has:
          2 with the Nationals
          5 with the Rays
          7 with the Astros
          3 with the Dodgers
          7 with the Yankees
          3 with the Royals
          6 with the Orioles
          3 with the Rangers
          3 with the Blue Jays
          3 with the White Sox

Not impossible but as a Sox fan i still dont like their chances. Especially the 10 games straight against the Astros and Dodgers

Meanwhile Seattle has the 4th easiest in schedule remaining in baseball. Boston if they are to stay in this they absolutely have to take advantage of this chaos and take the 5 against the Rays they also have to sweep the Royals, Yankees and White Sox. Then play 500 the rest of the games. They do that it’s 30 wins that should be enough….

Nope. I still love my team. injuries are a part of the game, and Wander…well, we don’t know for sure, do we? The facts will come out. If it is as bad as many on here think it is (and some seemingly want it to be–not you, but others), it will be awful–much more awful for the kid than for any Wander or TB fans. And guess what? I will still love my team and cheer them on.

I hope that no minor has been abused. But I am happy when the rich and famous are fully investigated and where appropriate prosecuted for heinous crimes where ordinary working folks would be jailed. No one is above the law. But if they just posed suggestively together, while dressed, it is very possible that the investigation could show abuse or also it could show no abuse at all. Let’s hope no one was abused but most importantly, let’s get it right.

For the record, Ruggs should have received a lot more and The RPWSRN should have gotten at least 10. You are right about our society being a clown show.

From what I have heard, the girl is now 18 and he is now 22. Which means he was 18 when she was 14. Not great in print, but basically the equivalent of a senior dating a freshman in high school. That happened in my high school and nobody thought anything of it other than the freshman was lucky enough to be dating a senior. Big difference than a 22 year old seeing a 14 year old.

But Blue, they’ve taken some serious blows. Their rotation is now Glasnow (recurring back issues), Eflin (recurring knee issues), Civale, and Littel (converted multi-DFA’d reliever). They may not make it into the playoffs, with the Astros, Blue Jays, and Mariners cruising up in their rearview.

The pitch clock should not have an effect. The pitchers do not throw any more pitches. They have had a pitch clock in the minors for years and many pitchers have had to deal with it already

The pitch clock makes pitchers rush their deliveries which messes up their mechanics causing injury. Any time you rush anything, it will have an adverse effect.

McClanahan would have had a pitch clock his entire MiLB career berfore reaching the Majors. He should have been well used to the tempo and mechanics should not be an issue for him . If it made that big of a difference, you would be seeing a lot of long time vets blowing their arms out as the long time guys never had to deal wit h a clock. There have been a ton of TJ surgeries before the clock was instituted.

          Idk about rushing their deliveries.

Throw the ball. Catcher throws it back. Get set. Go through your pitching motions. Throw the ball. That’s all doable in 30 seconds.

If anything it shortens recovery time between pitches. Guys throwing hard as they can or constantly throwing sliders curves that put strain on arms not able to recover as much would seem to be better bet.

          @Robertowannabe et al above…

I’ve got to agree – I don’t think the clock has any impact at all. If you go back (but post the original Tommy John surgery) the average game was done and dusted in about two and a quarter hours. (I’m currently enjoying watching the 1981 World Series) and more surgeries weren’t happening then.

Is it possible that the modern pitcher is a thrower rather than a pitcher?

(Caveat I’m English and a relative newbie to Baseball in 2012)

I agree. Back in the 70s when I was old enough to start paying attention to the nuances of the the game, If you had a guy that could get a FB into the 90s, he was throwing some serious heat,. The first guy that I remember hitting 100 consistently was Ryan. He was pretty much it for a lot of years. Now guys are routinely sitting in the mid to upper 90s with sliders and curves in the low 90s. Anything but a FB used to be in the low to mid 80s. Many modern pitchers are pitchers and not throwers but as you say the modern pitchers put a lot more stress and torque on their arms that are not meant to withstand that punishment. Their arms are breaking down much quicker now that they are hitting such high speeds on all of their pitches.

Tommy John. So the ligament gets replaced by a graft typically taken from one of the following tendons: Palmaris longus tendon from the forearm. Hamstring tendon. Big toe extensor tendon.

Are there stats somewhere on post surgery performance based on which tendon is grafted?

For these guys that need TJ twice does that mean the tendon graft ripped apart?

Can we double up going forward and put a combo toe/thigh or forearm/thigh graft together? Braid it up like a rope and make a super tendon?

Imagine deGrom getting the super braided tendon and throwing 120MPH into his 40’s……

Apparently, it can also be a donor tendon. The issue is the drilling of holes into the bone to attach them, not so much the tendon itself.

Can we just preemptively get the surgery done on pitchers before they hit the bigs? Or install robotic arms so if they break, they can be replaced for next game? Then guys could pitch every game with a robotic arm.

Old Yorkie: This is the 2nd Tommy John surgery for Shane Mcclanahan. He had it in college. It’s even mentioned in the article.

The Rays baby their pitchers as much as any team when it comes to pitch counts and innings limits, yet their entire pitching staff is decimated with arm injuries. I’ve never understood why teams restrict resilient young early 20 year olds from being stretched out and limit them, but they’ll ride a 30+ year-old far longer and push him to high innings and pitch counts. When I was 20, I could handle a lot more physically than I can now at 34. It just doesn’t make sense to me.

Tommy John Surgery is inevitable for anyone that is throwing hard and throwing often, so why not just cut the kids loose, accept that they will likely have the surgery at some point and then get them back afterwards?

Remember when the Nats shut down Strasburg in the heat of a pennant race to preserve the health of his arm? How that’s work out?

You mean when he came back and won a world series with them?

Seriously, does everyone on this board simply have no memory?

Correct – but they babied him, he needed TJS anyway despite babying him, he came back won a World Series, got a massive contract and has been dead ever since, and it has gone down as potentially the worst contract in MLB history.

But man, sure glad they held him back to save his health!

I remember the Nationals sabotaging the 2012 season and winning it all in 2019. Coming back….7 years later with a team that was almost completely different.

        You forgot that part.

Not only that, but I believe the pitcher that substituted for him had a great game. They didn’t lose because of Stras sitting.

Guess you didn’t watch the 2012 series against the Cardinals JoeB. What, Nats couldn’t have won in 4 if Strasburg pitched? Couldn’t have been up 2-0 in the series had Strasburg pitched?

Honest question: what is the point of competing, winning a World Series, or paying players millions to sit them out against their will? To stand by that decision to cost the team and paying fans a chance to win a World Series is nothing short of absurd. By your logic, Strasburg should have sat 2019 to stay healthy in 2020 and beyond. Nats fans will always remember 2019. THAT is what you play for. Not to monitor innings.

Only losers get Tommy John surgery just once, now all the cool kids are getting it twice. It’s like they’re using some BOGO coupons.

From a golden boy to a Golden Girl in one fell swoop. Rays fans go from Shane-ing McClanahan to Rue-ing McClanahan.

Getting Civale at the trade deadline looms even larger with this news. Mcclanahan might as well take care of that flexor strain while he’s at it, unless TJS takes care of that as well, correct me if I’m wrong here.

Thr Rays don’t need to worry. They can claim a DFA guy with a double digit ERA and get a top 5 Cy Young candidate (or something along those lines).

Amanda I get your point. But give Tampa credit for taking Wander off the field that is all they can do at this point until we know more.

@MannyBeingMVP Actually, no. The only just action is to thoroughly investigate the accusations while allowing the player to continue with his career on the field, unimpaired. It’s an accusation, not a guilty verdict, and yet the Rays are behaving as if the player is guilty, behavior that’s fundamentally absurd.

The Baltimore Orioles rank 14th in pitching overall and have a 99.5% chance of making the postseason….once in, just like the Phillies of last season, they have as good a chance of running the table to the WS as anyone who makes the playoffs….after all the playoffs are made for players who step up when not expected to……..and all this after many years of being the leagues punchline for mediocrity……..the Red Sox and their fans can only look up at the Orioles and wonder what might have been…stick that in your beans and eat them as your typically very jealous of their success and your teams lack of……….

Dodgers should try to pry him from the Rays during the off-season. They can have Walker and Shane. Seems like something the Dodgers would attempt….

Mcclanahan is going to still be dirt cheap for the next few years. No reason the Rays are going to punt on him and sell low. Glasnow is probably shopped this off-season though.

If the Rays can get value, I wouldn’t be shocked if they ‘diversify’ and de-risk the injured pitchers. McClanahan, Springs and Ras all coming back at the same time is going to be a lot, and Springs is now untradeable with his contract. At least 3 guys who won’t play in 2024 will take up 40-man roster spots all off-season. Could be 4 if they decide to keep Cleavinger. Trading one of McClanahan or Rasmussen makes a fair amount of sense.
