Injury Notes: Syndergaard, Cueto, Strasburg

Noah Syndergaard is close to returning and might jump straight to the big leagues without another rehab assignment, according to Anthony DiComo of MLB.com. The fireballer has had no shortage of obstacles to deal with this season. He underwent Tommy John surgery in March of 2020 and started his first rehab stint in May of this year. Unfortunately, that assignment was stopped when Syndergaard experienced some elbow inflammation and he didn’t get back on the mound until late August. Due to the small amount of time remaining in the season, the plan then was for him to return as a reliever, rather than the lengthier process of getting stretched out to start. But that second rehab assignment was shut down when he tested positive for COVID-19.

Despite the fact that there are only two weeks left in the MLB season now and the Mets seem to be fading out of the playoff picture, Syndergaard’s form in the remaining games is potentially very important for both he and the team. The 29-year-old is about to enter free agency and is a candidate for a qualifying offer. The quality and quantity of his appearances in the next two weeks could impact the club’s decision about whether or not to hand him that qualifying offer, as well as Syndergaard’s decision about whether or not to accept it.

More injury notes from around the league…

  • Johnny Cueto is nearing a rehab assignment, per Maria I. Guardado of MLB.com. The 35-year-old has been out since September 1st with an elbow strain but is scheduled to throw a bullpen tomorrow. It’s unclear what the rehab plan for Cueto is, but there’s not a lot of time left in the season for him to be stretched out as a starter. The Giants have largely been able to weather the storm without him, clinging to a narrow lead in the NL West in spite of having the occasional bullpen game. The club has of late been using a four-man rotation of Kevin Gausman, Logan Webb, Alex Wood and Anthony DeSclafani. Cueto has been solid when healthy this season, throwing 112 1/3 innings with an ERA of 4.09.
  • The plan for Stephen Strasburg is for him to begin throwing in November and be ready for spring training, Nationals manager Dave Martinez tells Todd Dybas of Inside the Clubhouse. Despite the Nationals stripping things down at this year’s deadline, it seems the club may be attempting a quick return to contention, as evidenced by their holding onto Juan Soto and targeting MLB-ready returns in their trades. A big wild card in that plan will be Strasburg’s health. The 33-year-old is a dominant pitcher when at his best, as evidenced by his excellent 2019 season, wherein he threw 209 innings with an ERA of 3.32 and then added 36 1/3 postseason innings with an ERA of 1.98, helping the club win the World Series and garnering himself World Series MVP honors. It was on the heels of that incredible performance that the Nationals and Strasburg agreed to a seven-year, $245MM contract. However, since then, the righty has only been able to throw 26 2/3 innings due to various injuries, most recently going under the knife for neurogenic thoracic outlet syndrome.

The limit on active roster players expanded from 26 to 28 today, as the calendar flipped to September. Every team announced at least two additions to the big league club (some teams made three or more due to injured list placements). Here’s a recap of today’s spate of transactions:

Giants Place Alex Wood On COVID-19 Injured List

AUGUST 31: Cueto has been activated from the IL to start this evening’s game against Milwaukee, the team announced. Quintana has also been activated after reporting to the club, with Vosler and Brebbia optioned in corresponding moves.

AUGUST 30: The Giants announced they’ve placed starting pitchers Alex Wood and Johnny Cueto on the COVID-19 injured list. Infielder Jason Vosler and reliever John Brebbia have been recalled from Triple-A Sacramento to take their places on the active roster.

Wood has tested positive for COVID-19, manager Gabe Kapler told reporters (including Alex Pavlovic of NBC Sports Bay Area). Cueto has not tested positive, but he’s feeling virus-like symptoms. The Giants will conduct an impromptu bullpen game tonight against the Brewers in place of Cueto, who had been scheduled to take the ball. The team remains hopeful Cueto could be back as soon as tomorrow.

Regardless of Cueto’s status, the Giants will be without Wood for the near future. San Francisco’s rotation depth is one of the team’s few question marks, particularly after Tyler Beede recently suffered a serious back injury in Triple-A. The Giants’ top five starters of Wood, Cueto, Kevin Gausman, Anthony DeSclafani and Logan Webb has remained remarkably healthy for much of the season. Sammy Long and the since-released Aaron Sanchez are the only other pitchers to log at least ten innings out of the San Francisco rotation.

Long could be an option to shoulder a heavier workload over the coming days, and San Francisco did just add José Quintana off waivers from the Angels. Matt Shoemaker is on hand as a potential option in Sacramento, but he’s struggled since signing a minor league deal earlier this month. San Francisco can ill-afford any sort of letup over the final few weeks of the season. While the league-best Giants are a lock to make the playoffs, they hold just a two and a half game lead over the Dodgers in the National League West.

Giants Place Johnny Cueto On Injured List

The Giants have placed starter Johnny Cueto on the 10-day injured list, retroactive to August 9, with a right flexor strain. Infielder Thairo Estrada has been recalled from Triple-A Sacramento to take his place on the active roster. Cueto’s injury is minor, manager Gabe Kapler told reporters (including Kerry Crowley of the Bay Area News Group), with the team hoping he’ll return after missing just one start.

Cueto has rather quietly put together a solid season for the first-place Giants. The 35-year-old has tossed 104 innings over nineteen starts, working to a 3.89 ERA/4.22 SIERA. That’s an unexpected bounceback since the two-time All-Star struggled in 2020 after missing large chunks of the 2018 and 2019 seasons recovering from Tommy John surgery. Cueto has seen a bit of an uptick in his swinging strike rate relative to the past few seasons, and he’s had some of the best control of any pitcher in the league. The righty’s 5.5% walk rate is fourteenth-lowest among the 78 hurlers with 100+ innings pitched this year.

With Aaron Sanchez designated for assignment over the weekend, the Giants’ rotation depth has thinned out in recent days. Anthony DeSclafani is expected to return from the IL when first eligible to start on Friday, Kapler told reporters. He’ll rejoin Kevin Gausman, Alex Wood and Logan Webb in the starting five. The Giants don’t have an off day until next Thursday, so they might have to turn to Sammy Long or Tyler Beede to cover for Cueto’s missed time over the next week.

Giants Make Several Roster Moves

The Giants made a number of roster moves ahead of tonight’s game. The biggest bit of news is the return of Mike Yastrzemski, who has been out since April 26th. While he’s not in the lineup tonight, he is available off the bench and should return to the lineup in the next couple of days. The 30-year-old outfielder put up a .215/.303/.468 line before straining his oblique.

With Yaz coming off the injured list, Alex Dickerson is heading the other way. Dickerson lands on the injured list with a bone bruise on his shoulder. Coming off a 150 wRC+ over a 52-game stretch in 2020, Dickerson has stumbled to a .213/.280/.347 line to start the season (79 wRC+). His strikeout rate is up from 17.6 percent to 22.0 percent, and his walk rate is down from 9.4 percent to 6.1 percent. He’ll get an opportunity to reset after recovering from this shoulder impingement.

Jarlin Garcia has also been added to the active roster taking Joey Bart’s spot after he was optioned to Triple-A, per Susan Slusser of the San Francisco Chronicle (Twitter links). Garcia had been out with left knee tendinitis. He surrendered six earned runs over 6 1/3 innings in five appearances early this season. All in all, it was a mixed bag for the 28-year-old southpaw, who managed a strong 32.1 percent strikeout rate despite serving up a pair of home runs and struggling with his command to the tune of a way-too-high 17.9 percent walk rate.

Finally, Johnny Cueto will return to the rotation on Sunday, and with Kevin Gausman set to return after his second vaccine shot, Logan Webb is likely headed to the bullpen. The Giants have been planning to use him as multi-inning reliever out of the pen, but injuries to Cueto and Alex Wood have kept him in the rotation to start the year. The Giants will need to make a roster move to bring Cueto back, with Sam Selman and Zack Littell being possible candidates for demotion, writes Slusser.

West Notes: Giants, Mariners, Athletics

Although outfielder Mike Yastrzemski suffered a mild oblique strain Sunday, the Giants are hopeful he’ll avoid the 10-day injured list, manager Gabe Kapler told Susan Slusser of the San Francisco Chronicle and other reporters. The Giants expect Yastrzemski to miss five to six games, but if he’s not ready beyond then, an IL placement may become necessary. Yastrzemski entered 2021 off back-to-back highly productive campaigns, and though he struggled at the outset of this season, his numbers began turning around before his injury. He has slashed .215/.303/.468 with four home runs in 89 plate appearances.

Here’s more regarding San Francisco and a couple other West Coast franchises:

  • The Giants put right-hander Johnny Cueto on the IL on April 15 with a Grade 1 lat strain, and he’ll sit out for at least a couple more weeks. Kapler revealed Cueto won’t return until May 9 at the earliest, which means he’ll wind up missing at least four starts, Slusser notes. San Francisco’s expectation was that Cueto would only miss two turns when he landed on the shelf. When healthy, Cueto has contributed to what has been an excellent Giants rotation with 20 innings of 1.80 ERA ball and a 24.3 percent strikeout rate against a 5.4 percent walk rate.
  • The Mariners have placed lefty Nick Margevicius on the 10-day IL with left shoulder inflammation and recalled righty Robert Dugger, the team announced. Margevicius’ IL trip comes one day after a disastrous performance in Boston, where he gave up four earned runs on two hits and four walks in a third of an inning. In five appearances (three starts) and 12 innings this year, Margevicius has been tattooed for 16 runs (11 earned) on 13 hits and seven walks. The Mariners are likely to plug righty Ljay Newsome into their rotation to take Margevicius’ place, manager Scott Servais suggested (via Ryan Divish of the Seattle Times). Newsome has held his own across four relief appearances this season, having allowed two earned runs on nine hits with 11 strikeouts against one walk over 10 2/3 frames.
  • The Athletics activated Mike Fiers from the IL on Monday, but the longtime starter will work out of their bullpen for the time being, manager Bob Melvin informed reporters (including Martin Gallegos of MLB.com). That means the A’s will stick with a five-man rotation consisting of Sean Manaea, Chris Bassitt, Frankie Montas, Jesus Luzardo and Cole Irvin. Fiers – who missed the first few weeks of the season with a lumbar strain – will try to bounce back from a disappointing showing 2020 showing in which he notched a 4.58 ERA/5.41 SIERA in 59 innings.

Injury Notes: Pham, Cueto, Flores, Yaz, Springer

Tommy Pham came up limping after grounding out in the second inning of tonight’s Padres game in Los Angeles, per AJ Cassavell of MLB.com (via Twitter). Pham stopped running about halfway down the baseline, clearly uncomfortable. Pham’s reaction, while muted, also suggested that he knew something was off as he reached for his ankle. An Achilles injury is the fear, but the injury appears to be left calf tightness, per Kevin Acee of the San Diego Union-Tribune (via Twitter). As for tonight, Jorge Mateo took over in left for Pham. Elsewhere…

  • Johnny Cueto is likely to miss a third start before returning from the injured list, per Susan Slusser of the San Francisco Chronicle (via Twitter). Cueto was placed on the injured list ten days ago with a grade 1 lat strain. The expectation at the time was that he would miss two starts, so it sounds like he’ll miss that target. That said, there was no indication that the injury was any more severe than originally diagnosed. Cueto should still be able to return shortly. In the meantime, Logan Webb will get at least one more turn in the rotation. Complicating matters in a good way for the Giants, Webb went seven scoreless against the Marlins today.
  • Also in San Francisco, Wilmer Flores missed today’s ballgame because he was under the weather, but after receiving fluids he’s on the mend. Mike Yastrzemski, meanwhile, was taken out of the game because of left oblique tightness, per the Athletic’s Andrew Baggarly (via Twitter). Because of their short bench, don’t be surprised if Yaz lands on the injured list tomorrow, but the team gave no indication one way or the other.
  • Blue Jays fans may finally get the chance to see offseason acquisition George Springer this week. Per Ben Nicholson-Smith of Sports.net.ca (via Twitter), manager Charlie Montoyo said, ““I’m optimistic that Springer’s got a good chance on Tuesday.” Springer’s return still hinges on his a final check-up, but the club hopes the 31-year-old will be in the lineup to face Max Scherzer in Dunedin on Tuesday.

Giants Place Johnny Cueto On 10-Day IL

APRIL 16: The Giants expect Cueto to miss two starts, according to manager Gabe Kapler (via Slusser).

APRIL 15: The Giants will place right-hander Johnny Cueto on the 10-day injured list with a Grade 1 lat strain, Susan Slusser of the San Francisco Chronicle was among those to report. They’ll make a corresponding move when it becomes official on Friday.

This will temporarily disrupt what has been an impressive rebound effort from Cueto, a former ace who didn’t pitch well in either of the previous two seasons. The 35-year-old has turned back the clock in the early going, though, having recorded a 1.80 ERA with a roughly average strikeout percentage (24.3) and a terrific walk rate (5.4 percent) through 20 innings. Cueto has also allowed just 13 hits – including three during a scoreless, 5 2/3-frame outing against his former team, the Reds, on Wednesday. He had to leave that start after only 68 pitches because of his injury.

The Giants do have a ready-made replacement for Cueto in free-agent pickup Alex Wood. The left-hander is scheduled to make his debut Sunday after missing the first couple weeks of 2021 while recovering from back surgery. Wood was supposed to bump Logan Webb to the bullpen, but both now figure to take spots alongside Kevin Gausman, Anthony DeSclafani and Aaron Sanchez in the Giants’ rotation.

Latest On Giants’ Rotation

Alex Wood will be activated from the injured list to make his Giants’ debut, starting Sunday’s game against the Marlins, per Alex Pavlovic of NBC Sports. Wood’s contract has a unique incentive structure that will pay out an additional $150K after 12 games of recording 10 or more outs. He receives more bonus cash after 14, 16, 18, 22, and 24 such games. Starting games certainly gives Wood the best chance to hit those markers, though health will be the biggest factor for the veteran southpaw.

Because of injures, Wood hasn’t been a regular rotation arm since back-to-back 150-inning seasons with the Dodgers in 2017 and 2018. Per Spotrac, Wood has seven career trips to the injured list, twice spending more than 100 days on the shelf. Arthroscopic debridement surgery on his left elbow limited him to 60 1/3 innings during the 2016 season. A back injury tanked his lone season with the Reds. He made just 7 starts covering 35 2/3 innings that year. Wood’s back has again given him problems this season.

With Wood joining the rotation, Logan Webb will head to the bullpen. Webb earned his rotation spot with a strong spring, but he’s off to a rough start. The 24-year-old has allowed 15 hits and five walks in just 10 1/3 innings in his two starts. While the move certainly may seem like a demotion, Pavlovic points out that Webb will be just the third right-hander in the Giants’ bullpen.

Of course, given today’s apparent injury to Johnny Cueto, the Giants may need to keep Webb in the rotation after all. Cueto left today’s game with a tight lat, per Kerry Crowley of the Bay Area News Group (via Twitter). Cueto has been excellent on this young season, tossing 8 2/3 innings of one-run ball in his season debut against the Rockies. He got his second win on the season today against the Reds. The veteran logged 5 2/3 scoreless innings before being removed because of the injury.

NL West Notes: Oberg, Rockies, Bumgarner, Cueto

Rockies reliever Scott Oberg is on the 45-day injured list due to blood clots in his throwing arm, marking the third time blood clots have interrupted Oberg’s career.  It remains to be seen if Oberg will be able to return during the 2020 season, though the right-hander told MLB.com’s Thomas Harding that he fully intends to keep pitching.  “If this is what it’s going to have to be, it’s just another hurdle to overcome.  I’ve dealt with the adversity road plenty of times,” Oberg said.  After already undergoing three different procedures in the past, it isn’t yet certain if a fourth surgery will be required; Oberg has consulted with doctors and specialists about his latest issue, with more consultations planned for the coming week.

  • With Oberg and the struggling Wade Davis both on the IL, the Rockies’ search for a closer is still ongoing.  It seemed as if Jairo Diaz was laying claim to the job, though after a rough outing (three walks, one hit, and two unearned runs in two-thirds of an inning) today, manager Bud Black told Patrick Saunders of the Denver Post and other media that the club has no “designated closer.”  It seems likely that Diaz will still get some save chances as part of the Rockies’ closing committee, though Yency Almonte, Carlos Estevez, Daniel Bard, and possibly others could all figure into the ninth inning depending on the situation.
  • Madison Bumgarner will throw a 50-pitch simulated game on Monday, a week after the southpaw was placed on the 10-day injured list due to a back strain.  Diamondbacks manager Torey Lovullo told reporters (including Nick Piecoro of the Arizona Republic) that the simulated game “is a way for us to see how he looks, see how he feels and have him evaluate how his body feels and give us that information and determine what the next step is moving forward.”  Piecoro notes that Bumgarner is still expected to miss at least one more start while on the IL, though the left-hander might not miss much beyond the 10-day minimum if he reports a clean bill after the sim game.
  • Could Johnny Cueto be turning himself into a trade candidate?  With the Giants in last place and looking like deadline sellers, John Shea of the San Francisco Chronicle writes that Cueto could get some attention from teams looking for veteran pitching.  Cueto has a 4.62 ERA, 1.73 K/BB rate, and 6.8 K/9 through 25 1/3 innings this season, some fairly middling numbers that ERA indicators (4.72 FIP, 5.55 xFIP, 5.28 SIERA) hint could or should be worse.  On the plus side, Statcast metrics show that Cueto is doing a good job of limiting hard contact, and Cueto is averaging 91 mph on his fastball, roughly the same average velocity he was posting in the couple of seasons prior to his August 2018 Tommy John surgery.  The Giants would likely have to eat some money to accommodate a trade, given that Cueto is owed $21MM in 2021, there is a $5MM buyout of his $22MM club option for 2022, and he is also owed the prorated remainder of his original $21MM salary for the rest of this season.
