COVID Notes: 4/16/21

Friday’s coronavirus-related updates from the majors:

  • The Astros placed second baseman Jose Altuve, third baseman Alex Bregman, designated hitter Yordan Alvarez, catcher Martin Maldonado and infielder Robel Garcia on the COVID injured list Wednesday. They’re unlikely to return for the team’s five-game road trip, manager Dusty Baker told Chandler Rome of the Houston Chronicle and other reporters. The Astros begin a three-game series in Seattle on Friday, and they’re scheduled to play in Colorado next Tuesday and Wednesday.
  • The White Sox announced that they have reinstated right-hander Dylan Cease from the COVID IL and optioned righty Zack Burdi to their alternate site. Cease was only on the list for two days, but he’s already set to return after “a series of negative tests,” the team stated. He’ll make his third start of the season Saturday against the Red Sox. Cease logged a 3.86 ERA with nine strikeouts and six walks over 9 1/3 innings in his first two appearances.
  • Twins manager Rocco Baldelli revealed that he received a false positive test on Thursday, Betsy Helfand of the St. Paul Pioneer Press tweets. He isolated in his office and was then cleared after three or four more tests. The Twins aren’t out of the woods yet, though, as Baldelli said (via Do-Hyoung Park of MLB.com) that they’ve had “further COVID-related issues in the clubhouse” on Friday. He couldn’t offer further details.
  • The Phillies with go without three members of manager Joe Girardi’s staff because of COVID protocols, according to Todd Zolecki of MLB.com. Hitting coach Joe Dillon, third base coach Dusty Wathan and bullpen coach Dave Lundquist are all away from the team. First base coach Paco Figueroa will move across the diamond to third, and coaching assistant Bobby Meacham will take over at first, Matt Breen of the Philadelphia Inquirer reports.

Astros Place 5 On Injured List

The Astros’ offense is taking a sizable hit, as the team announced Wednesday that designated hitter Yordan Alvarez, third baseman Alex Bregman, second baseman Jose Altuve, catcher Martin Maldonado and infielder Robel Garcia are all going to the injured list. The team made the moves due to health and safety protocols, according to Astros general manager James Click, who doesn’t expect their game against the Tigers on Wednesday to be postponed, Mark Berman of Fox 26 tweets.

Houston’s losing four regulars in Alvarez, Bregman, Altuve and Maldonado. The first three have been vital members of the Astros’ offense for multiple seasons, and all three have gotten off to strong starts this year. Maldonado isn’t nearly as effective at the plate, though the Astros like his defense enough that they had been finalizing a one-year extension with him as of Tuesday.

With such a large chunk of their position player group down, the Astros are bringing up infielders Abraham Toro, Alex De Goti and Taylor Jones, catcher Garrett Stubbs, and outfielder Ronnie Dawson. De Goti and Dawson weren’t on the Astros’ 40-man roster, so their contracts had to be selected.

Astros Notes: Bregman, Álvarez, Greinke, Javier

The Houston Astros will enter 2021 more vulnerable than ever atop the American League West. And yet, they remain atop the division. Or do they? Yesterday, our own Mark Polishuk asked MLBTR readers to predict the division winner, and so far, the Astros have fallen from their perch. Readers are currently projecting the A’s to win the division – though the Astros aren’t far behind.

Astros’ fever may spike as their stars return to health. Alex Bregman and Yordan Álvarez will both make their Grapefruit League debut on Tuesday, per MLB.com’s Brian McTaggart (via Twitter), which should provide a lift for manager Dusty Baker and company. Álvarez will be closely monitored for most of the season after offseason knee surgery. He’ll be limited to designated hitter duties in 2021. His bat, of course, can be a real difference maker. The 23-year-old owns an absurd .343 ISO though 378 plate appearances.

Bregman’s return to the lineup is no less crucial. Though he was the last arriving of the Astros’ original position player core, he’s grown into the best performer of the bunch. Back-to-back MVP-consideration seasons have launched Bregman into the first tier of MLB superstar. Especially with a roster that’s thinner that in year’s past, the Astros will need Bregman to ramp up to regular season form quickly.

When the season does open, it will be Zack Greinke on the hill. To no one’s surprise, Baker named Greinke their opening day starter, per Chandler Rome of the Houston Chronicle (via Twitter). Though he was a rotation luxury behind Gerrit Cole and Justin Verlander when he was acquired, Greinke has grown in importance over the past year and a half, not only on the hill, but in the clubhouse and in front of reporters as well. The veteran has a 3.54 ERA/3.02 FIP in 129 2/3 innings over two seasons since joining the Astros.

Elsewhere in the rotation, the Astros are already without Framber Valdez, and his replacement Jake Odorizzi won’t be ready by opening day. Cristian Javier, however, say he will be ready to go, per Sports Director at Fox 26 Mark Berman (via Twitter).

Injury Notes: Cain, Realmuto, Astros, Lowrie, Severino

Lorenzo Cain recently strained his calf in batting practice and is expected to miss one or two weeks, reports Robert Murray of FanSided. The injury is believed to be “very minor,” but the Brewers will be cautious in bringing the veteran center fielder back to action. Yesterday’s agreement with Jackie Bradley Jr. gives the Brewers cover at the position if Cain winds up needing more recovery time than expected. Manager Craig Counsell reiterated (via Murray) that Cain “is our center fielder,” so it seems Bradley’s ticketed for right field on most days if everyone’s healthy.

A few more injury updates:

  • Phillies catcher J.T. Realmuto had his hard cast removed and replaced with a splint as he works his way back from a fractured right thumb, Todd Zolecki of MLB.com was among those to report. That’s a generally encouraging sign, as Zolecki notes that this had been the anticipated next step in his recovery. However, Realmuto still has not progressed to a point where he can hit or throw. It remains unclear if he’ll be ready in time for Opening Day. Something resembling a typical season from Realmuto, arguably the game’s top catcher, is no doubt a key piece of Philadelphia’s hopes of snapping a nine-year playoff drought.
  • Third baseman Alex Bregman and designated hitter Yordan Álvarez haven’t yet gotten into spring training games for the Astros, but it doesn’t seem there’s much cause for concern. Bregman “tweaked” his hamstring and is taking things slowly, notes Brian McTaggart of MLB.com. The 26-year-old would be playing through the soreness if it were the regular season, he says. Álvarez is a little further from game action, per manager Dusty Baker (via McTaggart), although that seems to be mere caution after the young slugger underwent season-ending surgery on both knees last August.
  • Jed Lowrie is seeing action in simulated games but has not yet been cleared to play in spring training games, Athletics manager Bob Melvin told reporters (including Matt Kawahara of the San Francisco Chronicle). The former All-Star second baseman is back in A’s camp as a non-roster invitee after a two-year stint with the Mets that was ruined by injuries, most recently continued trouble with his left knee. Lowrie has taken just eight MLB place appearances since leaving Oakland after a stellar 2018 season.
  • Yankees right-hander Luis Severino continues to progress “really well” from his February 2020 Tommy John surgery, general manager Brian Cashman said this afternoon (via Erik Boland of Newsday). Severino had already moved to a long toss routine and soon should be able to throw off a mound, Cashman said. One of the best pitchers in the league from 2017-18, Severino has been limited to just 12 innings over the past two seasons by injury. His return would be a boon to a New York rotation that is high on upside but has plenty of risk behind ace Gerrit Cole.

Health Notes: Yordan, E-Rod, Bailey, Nottingham, Parra

After he missed all but two games last season, the Astros are set to welcome back slugger Yordan Alvarez this year. It appears the 2019 AL Rookie of the Year will be limited solely to designated hitter, though, as manager Dusty Baker said Friday (via Brian McTaggart of MLB.com) that it’s “very unlikely” he’ll play any outfield in 2021. Alvarez underwent surgery on both knees, and Baker commented, “It’s shown in the past that whenever [Alvarez] plays in the outfield, he’s hurting for two or three days, you know what I mean?” If Alvarez doesn’t see any action in the grass, it’ll leave left field to Michael Brantley, who battled his own injury troubles a year ago and spent most of his season at DH.

  • Red Sox left-hander Eduardo Rodriguez will return to the mound Saturday when he throws two innings in a simulated game, Chris Cotillo of MassLive.com relays. That’s welcome news after Rodriguez missed all of last year because of myocarditis and COVID-19. Rodriguez was one of Boston’s top pitchers during the previous couple seasons, and as a pending free agent, the 27-year-old is heading into a pivotal campaign.
  • Reds right-hander Brandon Bailey announced on Twitter that he underwent Tommy John surgery on Friday. It was the second TJ procedure for the 26-year-old Bailey, whom the Reds acquired from the Astros in November. The 26-year-old Bailey made his major league debut last season in Houston with 7 1/3 innings of two-run ball, six hits allowed, and four walks against three strikeouts. During his most recent minor league action in 2019, Bailey recorded a 3.30 ERA with more than a strikeout per inning across 92 2/3 Double-A frames.
  • Brewers catcher Jacob Nottingham is not recovering from late-December thumb surgery as quickly as expected. Nottingham is only doing “small baseball activities” right now and will not be ready for the start of Cactus League play, according to manager Craig Counsell (via Todd Rosiak and Tom Haudricourt of the Milwaukee Journal Sentinel). A healthy Nottingham and free-agent pickup Luke Maile figured to jockey for the No. 3 catcher position behind Omar Narvaez and Manny Pina, so Nottingham’s injury could help Maile in his quest to land that job. Nottingham, who is out of minor league options, amassed 54 plate appearances last year and hit .188/.278/.458 with four home runs.
  • Nationals outfielder Gerardo Parra underwent right knee surgery in the fall and is currently at 70 to 80 percent, he told Jesse Dougherty of the Washington Post and other reporters Friday. Parra expects to be ready for Opening Day, but having signed a minor league deal in the offseason, he has work to do this spring in order to earn a spot on Washington’s roster.

Yordan Alvarez Undergoes Surgery On Both Knees

It’s been known for more than a week that Astros slugger Yordan Alvarez would undergo surgery to repair a torn patellar tendon in his right knee, but the Astros on Friday announced that the reigning Rookie of the Year also underwent arthroscopic surgery on his left knee as well. That procedure is being termed by the Astros as a “routine cleanup.” The club’s press release indicates that the expectation is for Alvarez to “be ready to play prior to the start of Spring Training” next year.

Knee problems have plagued Alvarez, 23, throughout his professional career — even prior to his big league debut in 2019. On the one hand, it’s discouraging to hear of a dual knee procedure for such a young player, but on the other, the organization surely hopes that these operations will alleviate what has been a longstanding issue for a player who burst onto the scene as one of MLB’s best young hitters.

Alvarez didn’t make his Major League debut until June 9, 2019, but he still launched 27 home runs, ripped 26 doubles and plated 78 runs in just over half a season’s worth of games. Alvarez played in 87 contests last year and batted .313/.412/.655 over the life of 369 plate appearances. He won’t be eligible for arbitration until the 2022-23 offseason and can be controlled via that arbitration system all the way through the 2025 campaign.

Astros Place Yordan Alvarez On 45-Day Injured List

August 23: Alvarez was transferred to the 45-day injured list today, reports Brian McTaggart of MLB.com. Right-hander Chase DeJong had his contract selected for the 40-man roster, while Humberto Castellanos was optioned.

August 18: The Astros announced that DH Yordan Alvarez has been placed on the 10-day injured list (retroactive to August 16) due to right knee discomfort.  Infielder Taylor Jones has been called up from Houston’s alternate training site to take Alvarez’s roster spot.

Alvarez has appeared in only two games for the Astros this season, as he missed all of Summer Camp for undisclosed reasons and thus needed a few weeks to ramp up before joining the active roster.  Even once playing and even while homering and singling over his nine plate appearances, Alvarez was hampered by his knees, as he was a late scratch from Sunday’s lineup.  An MRI was negative, as Alvarez’s issues may simply be chronic.

“I asked [Alvarez] when did he start having these problems, and he said back in Double-A,” manager Dusty Baker told MLB.com’s Alyson Footer and other reporters yesterday.  “Most of the time when you have knee problems, they don’t subside very quickly. And when they do, they have a history of coming back.”

The Astros were only planning to use Alvarez only as a DH this season, and it seems possible that the 23-year-old might already be ticketed for DH-only duty at this early stage in his career if his knee problems prevent him from taking the field at first base or in left field.  That certainly puts a limitation on Alvarez’s overall value, though he has already shown a lot of talent at the plate — a whopping .313/.412/.655 slash line and 27 homers over 369 PA in 2019 earned Alvarez the American League’s Rookie Of The Year award.

Pitching injuries have been the story of the Astros’ season thus far, though the club is now missing two of its top bats with Alvarez and Michael Brantley both on the IL.  Jones, a 19th-round pick in the 2016 draft, may be more suited for bench depth than a regular role, though he did hit .291/.388/.501 over 531 Triple-A plate appearances in 2019 (albeit in the most hitter-friendly Triple-A season in history).  Jones made his big league debut earlier this season, appearing in one game for the Astros.

Yordan Alvarez To Undergo Surgery, Miss Rest Of Season

4:38pm: Baker announced Alvarez will undergo surgery on a partial tear of his right patella tendon and miss the rest of the season, Brian McTaggart of MLB.com tweets.

1:12pm: The Astros welcomed reigning AL Rookie of the Year Yordan Alvarez back for one game before knee troubles forced him from the lineup and eventually landed him on the injured list. Now, manager Dusty Baker tells Ross Villarreal of Sports Talk 790 in Houston that the slugger’s season is in jeopardy (Twitter link). “Yordan Alvarez, it looks like he’s done, maybe for the year,” Baker said of his designated hitter.

Knee troubles are nothing new for Alvarez, as he dealt with discomfort throughout his excellent rookie season and was hobbled during the original iteration of Spring Training as well. Details on the status of his knees have always been somewhat muddled, though. And as Chandler Rome of the Houston Chronicle points out (via Twitter), the Jeff Luhnow/A.J. Hinch regime and the James Click/Dusty Baker-led Astros have both been similarly vague when it comes to discussing Alvarez’s knees.

It’s been an injury-decimated season for the Astros. Justin Verlander has made just one start, while closer Roberto Osuna has been diagnosed with UCL damage. Brad Peacock and Jose Urquidy have yet to pitch. Right-hander Chris Devenski, Austin Pruitt and Rogelio Armenteros are all out with elbow troubles. On the position-player side of things, George Springer has been slowed by wrist troubles, and Houston has both Michael Brantley (quadriceps strain) and Aledmys Diaz (groin strain) on the 10-day IL at the moment.

Houston has reeled off six straight wins, boosting its record to 13-10 and surging back into the AL West race. They still trail the division-leading Athletics by two and a half games, but as the current second-place team in the AL West, they’re squarely in postseason position. The ’Stros have received huge performances from rookies Brandon Bielak and Cristian Javier in the rotation, which has helped to keep them afloat amid that barrage of injuries. Their schedule between now and the Aug. 31 trade deadline includes two games in Colorado, three in San Diego, three at home against the Angels and another three at home against the A’s. Houston’s play over the next two weeks will likely determine the deadline strategy, and that final three-game set against Oakland could be particularly pivotal in that regard.

AL Injury Notes: Yordan, White Sox, Donaldson, A’s

Astros slugger Yordan Alvarez was out of their lineup for the second straight day on Monday on account of right knee soreness, and it doesn’t seem as if a return to a regular role is imminent. While Alvarez’s MRI on Monday came back negative, he may only be a pinch hitter for the club in the near term, Alyson Footer of MLB.com writes. Alvarez’s lack of availability (he didn’t debut until Aug. 14 because of coronavirus-related problems) has been an enormous blow to a Houston team that has also dealt with several other significant health woes in the early going. The club lost yet another standout Saturday when it placed outfielder Michael Brantley on the IL with a right quad injury.

  • General manager Rick Hahn issued updates Monday on a pair of injured White Sox, second baseman Nick Madrigal and left-hander Aaron Bummer (Twitter links via James Fegan of The Athletic). The news on Madrigal is encouraging, as Hahn said the rookie has resumed “virtually all baseball activities” and remains on track to return by the end of this month after separating his shoulder Aug. 5. On the other hand, there’s no timetable for Bummer, who went to the IL on Aug. 8 with a left biceps strain. The groundball-heavy Bummer was off to a great start before then and seemed as if he was on his way to a second straight season as one of the game’s most effective relievers. Now, Bummer only has a little more than a month to return to regular-season action, and he’ll have to go through a throwing program before then.
  • Twins third baseman Josh Donaldson isn’t dealing with any issues in his recovery from a right calf strain, Betsy Helfand of the St. Paul Pioneer Press tweets. Donaldson went on the IL on Aug. 7, and it remains unclear when the big-money offseason signing will return to their lineup. The 34-year-old came out of the gates slowly prior to the injury, albeit over a mere 27 plate appearances, as he hit .182/.296/.318 with one home run. Nevertheless, The Twins have started 15-8 and sit atop the AL Central – a division they won last season.
  • Athletics right-hander Burch Smith headed to the IL over the weekend with a a forearm strain, which is always ominous for a pitcher. Manager Bob Melvin said Monday that Smith does not have any structural damage, but it’s up in the air whether he’ll pitch again in 2020, Matt Kawahara of the San Francisco Chronicle relays. Smith has been part of a slew of organizations, but he finally seemed to find a home in Oakland this season before the injury. The 30-year-old has logged a 2.25 ERA/2.30 FIP with 9.75 K/9 and 0.75 BB/9 in 12 innings from the A’s bullpen.

Injury Notes: Alvarez, Pham, Franmil, Judge

Yordan Alvarez was a late scratch from today’s Astros lineup, as the young slugger continues to battle knee issues.  Manager Dusty Baker told the Houston Chronicle’s Chandler Rome and other reporters that Alvarez arrived at the ballpark feeling knee soreness, which Baker suspected was from an awkward slide into third base during Saturday’s game.  Alvarez has been dealing with bad knees throughout his young career, and he has already been limited to DH duty for this season.

Since past tests haven’t revealed any structural problems, it could be that Alvarez’s knees could simply limit him going forward, perhaps both in 2020 and beyond.  It certainly isn’t what you want to see from a 23-year-old player, especially one who has been so productive even amidst this knee pain, and Baker acknowledged that the team will keep a close eye on managing Alvarez for the rest of the season.

More on some of the many injury situations popping up on a daily basis around baseball…

  • Tommy Pham was removed during a ninth-inning at-bat during today’s Padres/Diamondbacks game.  Pham was seemingly favoring his hand after hitting a foul ball, and left after a consultation with a team trainer.  This is the second straight game Pham has left early, as he was removed during the second inning of Saturday’s contest due to cramping in both calves.  However, he was back in today’s starting lineup as the DH and didn’t seem any worse for wear, going 1-for-4 with a walk and a stolen base. [UPDATE: the Padres are worried Pham has a broken hamate bone, Kevin Acee of the San Diego Union-Tribune reports.]
  • Franmil Reyes hit two home runs during the Indians’ 8-5 victory over the Tigers today, though Reyes’ big day had a painful ending after he was hit in the left hand by a pitch during a ninth-inning plate appearance.  Reyes was removed from the game, though he was diagnosed with only a contusion after x-rays were negative.  It seems likely Reyes will miss a game or two, though an injured list placement doesn’t seem likely at this point.  Reyes has been one of the few highlights for a struggling Cleveland lineup, as Reyes has hit .316/.366/.553 with five home runs.
  • Aaron Judge’s 10-day IL stint may be a minimal one, as the Yankees star told Newsday’s Roger Rubin and other media that he is already recovered from his minor calf strain just two days after his placement.  “I was begging for just a couple days…and I don’t need 10 days to be feeling good.  I already feel 100 percent now,” Judge said.  While Judge is “frustrated with myself not being out there,” he “wouldn’t say I’m frustrated with the organization.  This shows how much the organization cares for me and looks out for my well-being.”  Given how much time Judge (and other Yankees players) have missed due to injury in recent years, it makes sense that the club would be as cautious as possible in making sure a seemingly minor injury didn’t blossom into a bigger issue.
